At Lifepoint, we want your family and/or friends that have played a part in your decision to follow Christ, to be part of this special day. If you have someone that has been instrumental in your decision to follow Jesus and you would like for them to baptize you, please mark yes below.
The officiant of your baptism must be a member of Lifepoint Church or be approved by Pastor(s)/Elders.
If you mark no, the Executive Team will assign an officiant to baptize you.

The communicator or your story must be a member of Lifepoint Church or be approved by Pastor(s)/Elders.
If you mark no, the Executive Team will assign a communicator to read your story from stage.
Please indicate which service you would like to be baptized in.
We will provide a shirt for you on the day of your baptism. Please wear dark colored bottoms to maintain modesty.
We will reserve up to 15 seats for your friends and family to celebrate this special occasion with you!
